01 The problem with a cross
Twelve students, twelve different reasons
A class gets a percentage question wrong and the grade book records one fact: twelve crosses. Behind those twelve crosses are at least four different faults, and they need four different five-minute fixes, not one whole-class reteach of percentages.
Finding out which is which means reading twelve pieces of working and reconstructing what each student was thinking. It is the single most expensive thing a math teacher does, it is the first thing dropped when time runs out, and it is the thing an AI math tutor can actually take over.
- Base confusion. Applies the second percentage to the original number.
- Additive thinking. Assumes plus 20 and minus 20 cancel.
- Notation slip. Reads 20% as 20 and subtracts it.
- Arithmetic only. Method correct, one multiplication wrong.
Only the last of those needs practice. The first three need explanation, and they are invisible in a grade book.

Where a math misconception usually hides
The skill vocabulary is mathematical rather than generic, which is what makes the record usable in a data meeting. A few of the fault lines the diagnosis is built to separate:
Fractions
Denominator read as a count rather than a partition. Adding denominators. Comparing by numerator size alone.
Decimals and place value
Longer decimal read as larger. Misaligned columns. Zero treated as absent rather than as a placeholder.
Ratio and proportion
Additive reasoning where multiplicative is required. Part confused with whole in a ratio.
Percent
Base confusion in successive change. Percent of a percent. Reverse percentage treated as subtraction.
Algebraic manipulation
Distribution over a bracket applied to one term only. Sign errors when moving terms. Equals treated as a result arrow.
Geometry and measure
Area and perimeter conflated. Unit conversion applied linearly to an area. Diagram read as to scale.
03 The integrity question
Math is where answer engines do the most damage
Photograph-the-question solvers were the first mass-market AI in schools and they remain the most used. They produce a complete worked solution that is indistinguishable from understanding, which is why math homework stopped being a reliable diagnostic in a lot of schools.
The default here is the opposite. In Socratic mode the next move is a question and never contains the corrected value. Solver output
Final price: $38.40. Step 1: 40 x 1.2 = 48. Step 2: 48 x 0.8 = 38.40.
Correct, complete, and it tells the teacher nothing. The student copies it and the misconception survives to the exam.
Socratic output

The student has to do the multiplication and, more importantly, has to look at which number the percentage is of.
What the teacher gets either way
Both modes produce the same evidence card. Switching to a full worked explanation for a student who is genuinely stuck does not cost you the diagnostic record, which is why the toggle is safe to hand to a teacher rather than locked behind an administrator.
